Apply For a Grant

To be considered for one of our grants you must be a member of our “One Mission, One Force” partner network. Limit on funds requested is $20,000.00. Both public and private Oklahoma based organizations who serve veterans may apply. There are two funding cycles each year with deadlines being Dec. 31 and June 30. Organizations are limited to one grant per calendar year. (Force 50 Foundation grant funds will not be made for administrative costs including staff salaries and travel, or reoccurring operating costs.) * Denotes required field

Describe the program or project, and how a grant award not to exceed $20,000 from the Force 50 Foundation would be used.

Outcomes and Evaluation*(Required)

List the specific objectives and related outcomes of the project and how they will be measured. After grant funds are expended, agree to provide documentation that indicates how the donation was spent and, upon request, a testimonial from recipients who benefited from the grant.

Please attach: 1. A copy of your organization’s Internal Revenue Service Not-for-profit Determination Letter. 2. An Affirmation Letter signed by management stating that the IRS Determination Letter has not been revoked. 3. A copy of your current operating budget and financial statements, including salaries of key personnel. 4. A copy of your most recent financial audit and IRS Form 990, including all attachments. Note: (If your organization has been in existence for less than two years, a detailed business plan with a proposed budget will be required.) 5. A roster of your organization’s Board of Directors.
